Wayne County Health Department – Immunization Clinic

🕛 12:00 PM – 2:30 PM

The Wayne County Health Department will be onsite at the Jr./Sr. High School on October 10 to provide immunizations for students and staff.

Interested in participating?

  • Please complete the attached immunization form and return it to Emily Yost (District Nurse) prior to the clinic.
  • All students must have a Minor Consent Form signed by a parent or guardian.
  • For any questions regarding billing or costs, please contact the Wayne County Health Department directly.
  • Refer to the attached flyer for more details about available vaccines and eligibility.

Medication Administered at School
Use this form if your child requires medication during school hours. Physician and guardian signatures are required.

Back-to-School Vaccine Clinics

Clinics are provided by the Wayne County Health Department for students ages 4–18.

Demographic and Consent Form for Minors
Required for participation in vaccine clinics.
